1. How did you feel throughout the virtual experience?
Throughout the experience I felt like I was gaining a lot of insight into what Public
Health Nursing really looked like. The clinic experience was exactly what I
pictured when it came to Public Health Nursing. So I am glad that the VSim
matched with my thought process. Although, I think there is still a lot more that
Public Health Nurses do that I still haven’t witnessed.
2. What did you notice while completing this clinical experience?
I noticed that while I was conversing in the rooms with the patient's a lot of the
questions that popped up during the VSim I thought I knew what the correct
answer was, but I chose the wrong choice. Luckily I feel that I understand better
now due to the rationals provided in the simulation.

in managing transitions of care?
○ Reflect on the virtual experience and share insights into the role of
community/public health nurses in facilitating transitions of care for
clients. How did this experience enhance your understanding of
this role?
I learned that the role of a Public Health Nurse focuses a lot on case
management. Providing resources to the patient either through referrals or
tips to better their ambivalence of their health conditions. I also noticed
that a lot of the patients that were in the clinic had some knowledge of
their health conditions, but still required a lot of education. In order to
better facilitate transitioning care you need to focus on what information
and resources the patients lack in order to provide adequate care.

1. What did you learn about the complexity of working with a client who is
experiencing chronic illness management? What are the challenges of
clients who experience repeated hospitalizations, along with multiple
clinic visits?
I learned that clients often need more information regarding their health
conditions. I think that when juggling multiple chronic illnesses patients can get
confused when it comes to things like managing their medications. When it
comes to repeated hospitalizations patients may struggle with their health
maintenance, which causes exacerbations and a need to be hospitalized again.
Constantly needing to go to the hospital or visit the clinics can become an issue
with those who don't have access to appropriate finances, transportation, or have
a reliable care giver.
