Chief Complaint (CC):
A 66-year-old female presents with a 3-day history of
diarrhea.
History of Present Illness (HPI):
• Onset: Started 3 days ago, progressively worsening.
• Character: Loose, watery stools, occurring 6-8 times per
day.
• Associated Symptoms: Mild abdominal cramping,
occasional nausea, no vomiting, no blood in stool.
• Aggravating Factors: Worse after eating.
• Alleviating Factors: No significant relief with over-the-
counter loperamide.
• Recent Travel or Sick Contacts: None.
• Dietary History: Recently ate undercooked seafood at a
restaurant.
• Hydration Status: Feels weak and slightly dizzy, but no
syncope.

• Hyperlipidemia (HLD)

• Abdomen:
o Mild diffuse tenderness, no rebound or guarding.
o Normal bowel sounds, no distension.
• Skin:
o Dry mucous membranes (suggesting dehydration).
