1. Obtain a blank claim by making a copy of the CMS-1500 claim in Appendix I of the textbook or download
from the Student Resources at Cengage.com.
2. Review the Mary S. Patient case study (Figure 17-6).
3. Select the information needed from the case study, and enter the required information on the claim
according to the CMS-1500 claims completion instructions for workers’ compensation claims (Table 17-1).
4. Review the claim to be sure that all required blocks are properly completed.
5. Compare it to the completed claim in Figure 17-7.
