NR 601 Week 8 Reflection Amanda Zurlo Chamberlain University NR 601 Primary Care of the Maturing and Aged Family Practicum Dr. Dykes March 4, 2023 Describe how learning in this course helped you to meet Program Outcome 2: Create a caring environment for achieving quality health outcomes (Care- Focused). Provide at least two specific examples. This course helped me meet the outcome of creating a caring environment for achieving quality health outcomes by assessing the multitude of screening tools available for our patients in the office. One example was discussing the mental health screening tool we utilize in our clinical area. In my practicum location we utilize the Patient Health Questionnaire (PHQ-9). This questionnaire asks patients a series of questions about how they have felt over the past two weeks to gauge depression. This screening tool assists in achieving quality health outcomes by assessing for depression and if it is determined that the patient answers positively to any of the questions, further assessment and referrals are warranted. The next tool we discussed are screening tools to assess older adult patient safety at home. I discussed the Elder Mistreatment Screening and Response Tool (EM- SART). The EM-SART screening tool is three questions which are worded a little differently but still asking if someone close to them is harming them, taking their things against their will or failing to give the care they require. This assesses any risk for elder abuse. The other safety assessment tool discussed was The Lawton Instrumental Activities of Daily Living Scale (IADL). This screening tool is utilized to assess early functional decline and the ability to perform activities of daily living, which are very important to one's overall health, safety and level of function. Select one of the competencies from American Association of Colleges of Nursing (AACN) Essentials Domain 9 listed below and describe how learning in this course helped you to meet the advanced-level nursing education competency. Provide at least two specific examples. The competency I chose is 9.2: Employ participatory approach to nursing care. This course assisted me in becoming knowledgeable in the multiple consults/referrals to make in a multitude of situations. Through the i-Human in week 3, we discussed referrals for patients with thyroid dysfunction and addressed what to do in the situation if the patient has limited access to resources or cannot afford care. Also through each i-Human assignment we further researched follow ups and referrals for multiple diagnosis’ that we may see in the primary care setting. Also through this course, specifically in week 7 collaboration Cafe, we discussed inclusion when speaking about what to do in the community with a vulnerable and underserved population with education about palliative and hospice care. I discussed the ability to educate staff at the local clinics within the area that serves vulnerable populations, provide brochures for them to initiate the education and then offer to be on site to provide further education as well. Also having these brochures in multiple languages is important as well.
