change of one of the chosen types of support:

In this assignment I will explain the role of one health and social care professionals
who helps and supports the individual who is experiencing transition and change. I
will also talk about how three types of support may be used by the professional in
order to help the individual experiencing transition and change. Lastly, I will analyse
the benefits of the support from that professional.

The health and social care professional I have chosen is Social worker. The role of the
social worker is to help people and families who are going through difficult times.
Their main responsibility is to help people to solve issues in order to improve the
state of the individual in a positive direction. Social workers may work with different
groups of people depending on what type of support they need and how serious it is.
People like that could be elderly, people with mental disorders/disabilities, drug and
alcohol abusers, homeless people and many more.

A social worker can help and support those who are experiencing transition and
change by helping gain contacts and for example, if the person is homeless they
might try and help them find some sort of job and a place to stay whilst they are
unable to find their own home.

Social workers could help young people with disabilities to move into the adulthood.
One of the ways in which the social worker could help those who have disabilities, is
with the financial issues. This is one of the types of support which the individual
experiencing change could get. The benefit of that could be that the individual who is
experiencing change is more likely to afford the equipment they might need in order
to develop their skills in a positive way. This could also give the individual who is
experiencing transition to afford a therapist who is going to help them develop their
skills.
