Procedure Steps
Sterile, Wet-to-Dry Dressing Change
• Place supplies at bedside
• Set up trash receptacle in an accessible location
• Perform hand hygiene and don clean gloves*
• Raise bed to an appropriate height and lower side rail
• Position client appropriately to access the wound
• Place absorbent pad under client
• Remove current dressing to expose wound
• Remove packing from wound
• Assess wound appearance and appearance of any
drainage on the dressing and packing
• Measure wound’s dimensions using wound measuring
guide and cotton tipped applicator
• Discard guide and applicator
• Remove gloves and perform hand hygiene*
• Prepare supplies for cleaning and dressing wound
following appropriate sterile technique*
Open gauze pads in tub
Open single gauze pads
Open absorbent ABD pad
Moisten gauze pads in tub
• Don sterile gloves*
• Clean wound working from cleanest to dirtiest*
• Pack wound gently with moist gauze*
