NURS 3460 Foundations of Adult Health Exam 1 Blue Print – 2021 EXAM One Blue Print Chapter 1, 6, 16-20, & 29: Know how to perform assessment of pulse pg. 497 - You can assess any artery for pulse rate, but you normally use the radial artery because it’s easy to palpate. - Tachycardia: pulse rate faster than 100 - Bradycardia: pulse rate slower than 60 - Infant: 120-160, Toddler: 90-140, Preschooler: 80-110, School-age child: 75-100, Adolescent: 60-90 - Adult: 60-100 (normal) Sites: - Temporal, Carotid, Apical, Brachial, Radial, Ulnar, Femoral, Popliteal, Posterior tibial, Dorsalis pedis. Radial Pulse: - Perform hand hygiene - Provide privacy - Verify patient’s identity - Ask if they have any allergies - Have patient in a supine or sitting position, if supine make sure arm is alongside the body, if sitting have patients arm at a 90-degree angle and support the lower arm on chair or your arm. - Place tips of fingers along the radial of thumb side of patient’s inner wrist. - Lightly compress against the pulse - Note characters of pulse (rate, rhythm, strength, equality) - Count for 30 seconds and multiply by 2 if normal, and a full minute if irregular - If needed to: o Lower bed back to how it was and give patient call bell and their electronics o Perform hand hygiene before exiting room Apical Pulse: - Perform hand hygiene - Provide privacy - Verify patient’s identity - Ask if they have any allergies - Clean stethoscope - Have patient in a supine or sitting position - - -
