Cynthia Francis is a 56-year-old obese female with a PMH of hypothyroidism who
presents for a blood pressure recheck. Three weeks ago, she attended a workplace
health fair where her blood pressure was measured at 132/74 mmHg. The nurse
advised her to follow up with a primary care provider within 4 weeks for re-evaluation.
She has not been monitoring her blood pressure at home .
She recently moved to the area approximately 5 months ago and has not yet
established care with a primary care provider. She reports:
Diet: High-fat, high-salt intake; eats dinner out 3-4 times per week; comfort foods when
stressed
Physical activity: Sedentary (desk job); no regular exercise; only brief walks twice
weekly
Alcohol: 1-2 glasses of wine on weekends
Smoking: Former smoker (quit 10 years ago; 1 pack/day for 15 years = 20 pack-year
history)
Symptom Review:
Denies chest pain, palpitations, shortness of breath, or leg swelling
Denies polyuria, polydipsia, polyphagia, weakness, numbness, or tingling
