Chapter 25: Acute glomerulonephritis: treatment, s/s, nursing interventions Immunologic disorder in the kidney proper; did not begin in the bladder and ascend Causes: o Primary event (Generally follows other bacterial illness such as Pneumococcal, streptococcal, or viral but especially strep) o Evidence of systemic disorder such as SLE and Sickle Treatment o General support measures, early recognition, and tx of complications o Pt with normal BP and urine output can be treated at home o Pt with complications i.e. edema, HTN, hematuria, and oliguria need hospitalization o General measures- limit activity but bed rest is not necessary o Fluid balance- monitor VS, I&O, daily weights, limit Na intake, diuretics if edematous or fluid overload, in severe cases, peritoneal dialysis or hemodialysis, HTN control by loop diuretics or CCB, BB, or ACE I., anticonvulsant therapy if pt has hypertensive encephalopathy o Nutrition- limit foods high in Na, don’t add salt to food, limit foods high in K o Antibiotics- ONLY if presence of persistent streptococcal infections. Abx do not really change patho of disease but can help prevent spread to family
