Service Line for Johns Hopkins Mobile Heart Failure Clinic
Proposed Nursing Service Idea
The proposed service line is a mobile heart failure clinic through
Johns Hopkins Hospital (JHH). This service would be able to provide
follow-ups and as-needed visits to those with difficult, limited, or no
transportation. Like home health agencies bringing healthcare to those at
home, this mobile clinic would bring the heart failure clinic to the patient
through the capabilities and skills of an RN and APP (advanced practice
providers such as NP or PA), Monday through Friday, between the hours of
7 am and 5 pm. Much like the traditional clinic appointment, a dry weight
and a full set of vital signs would be obtained. The RN would verify the
patient’s current medications and the APP would assess and treat the
patient accordingly. Basic labs would be run as a point-of-care test finger
stick and diuresis could be done in-home with the establishment of an IV.
In order for this service line to be established, it will need to be
spearheaded by someone not with exceptional organizational skills.
Because the mobile clinic would initially get most of its patients through
referrals from inpatient care teams, interpersonal skills will also be
paramount in building those working relationships between the teams and
the mobile clinic. The leader will also need to demonstrate the ability to be
flexible and adapt. As a brand-new start-up, the mobile clinic will have its
faults and setbacks. An adaptable and flexible leader will be able to collect
and interpret feedback, make needed changes, and follow up to ensure the
changes made are effective. Lastly, the leader will need to function in the
democratic and authoritarian realms due to this being such a new service.
With the hope of hiring staff with cardiology and/or home health
backgrounds, a democratic leader can take the shared experience of the
staff in order to make decisions or changes. However, a more authoritarian
approach may need to be used in the formation of processes and structure
of the mobile clinic.

Importance
According to the 2010-2015 community health assessment, heart
disease accounts for 24.4% of deaths in Baltimore City, more than all
cancers combined (Baltimore City Health Department, 2017). This service
could eliminate the transportation barrier for those with limited or no
transportation available and help increase their quality of life.
