History of Present Illness (HPI)
The patient is a 25-year-old female who presents with a 4-day history of loose stools. She reports
4–6 watery bowel movements per day, non-bloody and without mucus. Symptoms began
abruptly after consuming street food at a local market. She endorses mild, intermittent lower
abdominal cramping relieved by defecation. She denies fever, chills, vomiting, nocturnal
diarrhea, recent antibiotic use, or recent travel. Oral intake has been maintained with mild nausea
but no vomiting. No prior history of similar episodes.
