Chief Complaint (CC)
“Frequent headaches for the past few weeks.”
, History of Present Illness (HPI)
The patient is a 25-year-old female who presents with complaints of recurrent
headaches for approximately 3–4 weeks. She describes the pain as a bilateral,
dull, pressing, band-like sensation around her head. The headaches typically begin
in the late afternoon and gradually intensify but do not reach severe intensity.
Pain is rated 4–6/10.
The headaches are not associated with nausea, vomiting, photophobia,
phonophobia, aura, visual changes, weakness, dizziness, or loss of consciousness.
Symptoms do not worsen with physical activity and are relieved with rest and
over-the-counter acetaminophen.
The patient reports increased academic and work-related stress, poor sleep
quality, and prolonged screen time. She denies recent head trauma, fever, neck
stiffness, or infection. There is no history of similar severe headaches in the past.
Past Medical History (PMH)
Generally healthy
No history of migraines or neurologic disorders

None
