,Shortly after discharge from a psychiatric unit, a patient is brought back to the
emergency room for recurrence of manic symptoms. The patient states that their
symptoms were a punishment from God for having thoughts of sexual impropriety,
and that at their family's urging had stopped their medications and began attending
religious services to pray for forgiveness. According to the Outline for Cultural
Formulation in the DSM, these symptoms are best classified as part of the patient's:

A. cultural identity of the individual
B. culture-bound psychiatric syndrome
C. cultural conceptualization of distress
D. cultural features of vulnerability and resilience
E. cultural understanding of the treatment relationship
cultural conceptualization of distress

Once referred to as "culture-bound syndromes," these culturally rooted groups of
symptoms are now known as "cultural concepts of distress."

A 25 year old patient with no previous psychiatric history has a new preoccupation

does not have other psychiatric symptoms. Which of the following medications is
most appropriate for the patient?

A physician recommends that an outpatient start buprenorphine for opioid use
disorder. The patient admits to last using heroin an hour before the appointment.
The physician should initiate buprenorphine:

A. after a negative urine toxicology test
B. when acute intoxication has worn off
C. when the patient is acutely intoxicated
D. when the patient is experiencing mild withdrawal
E. once the patient is no longer experiencing any withdrawal symptoms
when the patient is experiencing mild withdrawal

Those starting buprenorphine must be in a state of withdrawal. Buprenorphine
could precipitate withdrawal if given to an opioid-dependent patient experiencing
opioid effects.

Use an opioid withdrawal scale (e.g. Clinical Opiate Withdrawal Scale [COWS])
to determine if patient is experiencing mild-moderate symptoms of withdrawal to
avoid risk of precipitated withdrawal.4 Some recommend waiting until a COWS
score of 6-10 or more is observed before buprenorphine is started;5 others suggest
a score of 11-12 or more.2

Buprenorphine acts as a partial mixed opioid agonist at the μ-receptor and as an
antagonist at the κ-receptor. It has a higher affinity for the μ-receptor than other
opioids, and it can precipitate withdrawal symptoms in those actively using other
opioids.
