Week 8: What Did You Learn Reflecting on this course, consider and answer the following questions: How do you envision using the AACN essentials and information learned in this course to identify, guide, and evaluate your practicum project? How do you envision using the concepts learned in this course in your future nursing practice? 1. What I Learned: The nine essentials from the American Association of College of Nursing (AACN) that were integrated into our learning in this class will definitely reflect in my practice when I start my practicum project, which will then reflect in my future nursing practice as a family nurse practitioner. The essentials are the core to my success as a Master’s prepared nurse pursuing the family nurse practitioner track. I learned skills to be a true leader, a scholar through evidence-based practice, the importance of a person-centered care, the importance of caring, importance of change in a workplace, and importance of workplace civility. All of this knowledge learned integrates into one another with the end result being providing the best care possible to my patients and their family members. As a family nurse practitioner, I must start with being a good leader for leaders are role models who establish the “moral tone” and are the “moral compass” for the environment (Ellis, 2016, p. 58). The environment is a reflection of the leader. If one’s environment becomes affected by incivility, it is because it was allowed. Self-reflection would allow for colleagues to understand the moral and ethical behaviors. Self-awareness reflects one’s beliefs, values, and feelings, for these attributes influence our behaviors towards others (Rasheed, 2015). As a scholarly nurse pursuing the family nurse practitioner track my priority is on patient-centered care, which incorporates holistic care, and treating individuals with the best treatment through evidenced-based practice. Evidence-based practice leads to change in a health care system; thus, it is understood how healthcare systems are complex environments continuously changing in order to evolve to reflect best practices. As a scholar, evidence-based practice is the process of researching best practices, learning, being inquisitive, and implementing the changes to yield positive patient outcomes (Stevens, 2013). One takes into consideration the entire person by integrating all aspects of their life and realizing their uniqueness and individuality, consisting of values, cultural beliefs, and mannerisms influenced by their environment, experiences, traditions and family (Kitson, 2004). As a master’s-prepared future family nurse practitioner, I would like to push myself to strengthen my leadership skills as well as provide a treatment plan that is safe and effective for any all and all patients that are under my care. Applying the AACN essentials learned in this course along with the future courses, will be used as a guide to enhance my knowledge and as a reference tool, all the master’s essentials will continue to be helpful throughout the duration of my career. As a master’s-prepared nurse, I will be equipped with valuable knowledge and skills to identify, evaluate, and treat cases that are presented. Week 8: What did you learn I was expecting a very basic introduction class for getting back into school but lo and behold I learned some things! The discussion questions have already provided me with guidance for a subject of what I would like to do for my practicum project and reviewing the number of professional, peer reviewed journals has provided glimpses into how the research can be conducted and the types of conclusions I will be expected to provide at the end of the project. Research has always been my weak point in school, finding and referencing information is tedious and does not hold my attention the same way it does some others and while I enjoy writing papers on subjects that I have passion and interest for, finding the research and supporting information is time consuming and difficult. This class helped me to overcome that view a little, forcing me to use and explore research resources and gain an appreciation for them. The AACN concepts that we explored through these weeks will help me to create an educational concept for student nurses that has a founding in a greater number of areas needed for a well-rounded nurse. Each one will play a role in an advanced nurses’ job throughout his/her future career. As for me, I will utilize each one to the best of my ability to identify, guide, and evaluate my practicum project. I envision using the AACN essentials as an outline for my practicum project. The first essential is about background. This is what can be used to identify and research on my practicum project. Essentials 2,3, 5, 6, and 7 can be used as a guide. For evaluation, essential four and nine can be used. These two essentials are about understanding, researching and putting things into practice. In the first discussion of this course, we discussed the value of a master’s prepared nurse and how education is important. In my future nursing practice, I envision continuously keeping myself educated. As stated in week 2, I want to implement policies that look at the patient physically, psychologically, sociologically, developmentally, spiritually, and culturally. Having good communication and utilizing the ten common professional nursing values to be a good nurse executive. Halfway through the course, we learned about leadership. As a future nurse executive, I envision having weekly meetings with staff to evaluate the floor and staff. Also, I envision pursuing the topic discussed in week 5 about chronic disease patients returning to the hospital for the same condition within 30 days. In week 6, we learned about complex adaptive system. I believe that I can use this system as a future nurse executive. And lastly, I want to make sure as a future nurse executive that any form of incivility, bullying, or horizontal violence does not occur. If it does occur, I envision taking disciplinary actions to the people in questions. Being an online master’s student is very different from being in a classroom. We as students still learn the requirements for obtaining a master’s, but differently. Studies were done on online graduate nursing students and their knowledge, attitude, and practice of evidence-based practice. In the article, Evidence-based practice knowledge, attitude, and practice of online graduate nursing students, it discussed the study of online graduate nursing students taking a course. The study showed the before and after effects of taking a course with evidence-based practice in it. The result of the study was that their knowledge increased. One believes that this course has increased the knowledge of a bachelor’s nurse into transitioning into a master’s prepared advanced practice nurse (Rojjanasrirat & Rice, 2017). The AACN Master’s Essentials helped to address and clarify the skills necessary to deliver high-quality, patient-centered care. Initially, I was terrified as to what to expect for this course considering I am torn between leadership and family nurse practitioner, you have your mind set and ready to conquer. Before taking this course, I never stopped to think about what being a “true leader” entails. I am now integrating the leadership skills we discussed into my work ethics, for I want to be a an influential role model to those around me in order to maintain a healthy environment. I learned that self-reflecting on issues such as biases, body language, speech, and ect, influences all aspects of life including the relationship with colleagues and patients. The AACN and CCN guidelines help to identify how each role is intertwined and that a collaborative approach to healthcare through multiple disciplines can improve outcomes for healthcare providers and recipients. This course has helped solidify the need for continuing education, research and the implementation of evidence-based practice. The AACN lists 9 essentials that are the core of all masters levels programs. Although all the essentials are equally important in advanced nursing practice, I plan on focusing on Essential II: Organizational Safety and Systems Leadership, Essential III: Quality Improvement and Safety, and Essential IV: Translating and Integrating Scholarship into Practice as I navigate my practicum project. After collaborating with my future mentor and identifying a specific need in the facility I will perform my practicum. I am hopeful to focus my practicum on the importance of shared governance and it’s relationship to healthy work environments and subsequent improved patient and employee outcomes. The nurse executive is essential in ensuring the provision of healthful work environments by enforcing the standards of the AACN, evaluating the environment through administration of satisfaction surveys, re-evaluating process improvements, and modeling appropriate behavior. The AACN (2015) shares that in order to facilitate positive change, leaders need to have vision and appropriate management tools that engage personnel (para. 1). The nurse executive committed to providing authentic leadership should exhibit the same behavior and values expected from their employees. Self- reflection of the nurse executive is an important tool to evaluate personal behavior traits and develop corrective action plans. The Association of Nurse Executives (2016), identified 9 elements that foster healthy work environments. These elements are similar to the ones identified in this paper by the AACN (2015), but elaborate on the nurse executives’ role in recognizing and rewarding employees, promoting education and certification, and supporting nurse involvement in decision-making (para. 3). As a leader of the healthcare team, pursuit of the actions above will lead to nurse satisfaction rates, high quality care, and a culture of safety which all lead to healthy work environments. Empowering, engaging, and rewarding personnel should be a top priority for the nurse executive in cultivating this culture. The two that will stick out the most for me in the speciality track I have chosen is essential V: Informatics and Healthcare Technologies and essential VII: Interprofessional Collaboration for Improving Patient and Population Health Organization. Informatics and Healthcare Technologies because with the new technology that is being introduced into the healthcare as a nurse educator or instructor it is important that I stay informed of the changes so that I can teach others how to use the new technology that is pretty much taking over. Then I chose Inter professional Collaboration for Improving Patient and Population because I would really like the saying that nurses eat their young disappear and if in nursing school teamwork and collaboration is taught early then the new nurses can be force of change with that myth and prove that working together to achieve a common goal if what patient care is all about. The one concept that I plan on using more is examining myself first especially when it comes to challenging positions or situations. Technology is expanding everyday and educators do need to stay up to date with these technologies in order to instruct their students correctly. I also have seen in the clinical setting that some nurses are intimidated by these newer technologies until they have a good understanding of their application. Essential VII was also a great one to identify, especially in terms of nurses of varying expertise working together instead of tearing each other down! Manage Discussion Entry Class and Dr. Walker After 9 years of undergraduate classes the first class beginning my graduate program is coming to an end. The AACN essentials were introduced in week 1 and each week the class touched on an area that was represented in the document. In a graduate program emphasis is placed on theories and concepts of nursing application and science along with the management of health care systems and with a large emphasis on research development (AACN, 2011). This course had us consider how we would lead change in our respective tracks but also reflect on how it would impact different levels of the organization, and how the importance of reflection and taking care of ourselves and looking at our values and how we treat others and how not treating each other well can harm the whole environment. Reading about the importance of reflection and thinking about the 6 C’s of core values by Sister Ramoine Roach (1922-2016) compassion, competence, commitment, confidence and courage identified a theory that helped bring me to a new direction (Bailey, 2017). At the beginning of this class I was certain my direction would be the education track. As each week progressed and the more we looked inward and at our current practice I began to see that as much as I enjoy teaching new nurses I truly love patient care and after 3 more classes I may be switching to the FNP track. The AACN essentials states that the clinical experiences give the masters prepared nurse the didactic learning experience, that not only promotes innovative thinking but would allow the nurse to test new solutions to clinical practice system issues (AACN, 2011) Depending on whether I stay on the education track or switch to FNP I am certain that I will utilize what I have learned to guide my project. If I stay on the education track I still am very interested in improving preceptor education and if I am on the FNP track I am passionate about working with adolescents with eating disorders. The incivility paper really made me think deeply about the direction of my practice and look at my current place of employment and how a change needs to be made. It also has shown me that I should not stay silent and should possibly take my concerns up the chain of command. Seeing the divide in my unit concerns me and as I continue my practice finding ways to work together with a diverse workforce, one of not only differently races, cultures, and genders but also the generational difference is very important and will continue to influence my practice. Exploring the AACN essentials allowed the class to consider how these essentials can be utilized in practice. This author has gained insight and will utilize the AACN essentials in future practice. In identifying the practicum project it will now be in this author's selection, imperative that the project addresses at least a few of the AACN essentials. This criterion will also serve to evaluate the project at the end to ensure the AACN essentials were addressed. Many concepts were also addressed in this course. These concepts exist as a framework for this author. Having discussed incivility in nursing, biases, creating changes on the systematic levels, and the concept of caring has left this author with much to consider. Some of these areas were known and this author had already planned on utilizing these key points in practice. However, as a core framework for future practice, some of these concepts identified shortcomings that were addressed. Furthermore, having discovered this at the beginning of the graduate education journey, this author know has time to build and strengthen these areas in need of improvement. This will ensure a strong foundational core in this author's future nursing practice. This course has also prepared this author for future academic work. Studies have proven that EBP provides improved quality of care, safe practices, positive patient outcomes and decreased health care costs (Zimmerman, 2017). I plane to center my practicum project surrounding chronic illness and the impact EBP has on the outcomes of these patients. Evaluation process is unclear because criteria for the project have not been presented. Evaluation of the project could consist of scholarly evidence for information presented. How do you envision using the concepts learned in this course in your future nursing practice? Definitely, I will research the most recent EBP regarding chronic illness for my patients. These essentials will help me improve the quality in outcomes with the patients I encounter, advance my knowledge and learning, while helping me develop leadership skills and promote working as a team in my future career (AACN, 2011). The AACN essentials will guide my professional choice of master’s prepared nurse practitioner especially using the two essentials of advocacy and inteprofessional collaboration for improving patient and population health outcomes.
