Week 5 Case Study: Acute Monoarticular Arthritis (Gout)



S: Subjective

Chief Complaint (CC):
"My right knee is swollen and hurts a lot."

History of Present Illness (HPI):
Mr. Tod Gouseau, a 57-year-old male, presents with acute onset of right knee pain and swelling
that began two days ago. He reports the pain started suddenly, worsened overnight, and now
rates it as 8/10. The pain is sharp, throbbing, and exacerbated by movement or touch. He denies
trauma or overuse. He recalls a similar episode involving his left great toe one year ago, which
resolved spontaneously after a few days. He denies fever or chills. He consumed red meat and
alcohol (beer) over the weekend.

Associated Symptoms:

• Swelling
• Redness
• Warmth
• Limited range of motion
• Difficulty bearing weight

Medications:

• Ibuprofen PRN for pain
• No chronic medications

Allergies:

• NKDA

Past Medical History (PMH):

• Hypertension (not currently medicated)
