S – Subjective
Chief Complaint:
"My right knee is swollen and painful; it started last night."
HPI:
Antonio Cobb is a 65-year-old male presenting with acute onset of right knee pain and swelling
that began 24 hours ago. The pain is described as severe, sharp, and constant, rated 8/10, and
worsens with movement. No known trauma. He reports similar episodes in the past affecting the
big toe. He denies fever, chills, or recent illness. No over-the-counter or prescription medications
taken for the current episode.
PMH:
• Hypertension
• Hyperlipidemia
• Gout (previously diagnosed, poorly controlled)
• Obesity
• Chronic kidney disease (stage 2)
PSH:
• Appendectomy (age 25)
Medications:
• Lisinopril 10 mg daily
• Atorvastatin 20 mg daily
• No current urate-lowering therapy (stopped allopurinol 6 months ago)
Allergies:
• NKDA
