Chief Complaint (CC):
The patient reports feeling "depressed and anxious," with a significant increase in fatigue
and a lack of motivation over the past few months.
History of Present Illness (HPI):
The patient is a 42-year-old female with a past medical history of MDD and complains of
deterioration in her depressive and anxiety symptoms for the past 3 months. They outline the
symptoms that indicate low mood, hopelessness, powerlessness, reduced energy and social
interest, anhedonia, and inability to derive pleasure from previously enjoyed activities. The
patient also complains of increased worry and reduced ability to concentrate, hence a decline in
work productivity. They also suffer from sleeplessness, consequently waking up frequently
during the night, making it difficult for them to have a sound sleep that will help them avoid
fatigue in the coming day. The patient often does not follow the prescribed medications except
occasionally; they feel that their condition is worsening and personal functioning in daily
activities is becoming compromised.
They categorically respond in the negative concerning any recent alterations in the
pattern of substance use and suicidal thoughts. While they do not acknowledge any suicidal
thoughts in the recent past, they admit to having had occasional thoughts about hopelessness in
the past. The patient reveals various stress factors, such as financial threats and interpersonal
clashes at the workplace, that can explain the worsening of the symptoms.
