They hope to nurture public health by building contributions to havestrong public health leaders and capabilities to protect Americans from health threats (CDCorganization, 2020).The Associations Ability to Promote Equal Opportunity and Improve Life QualityIn 2015, CDC released the Community Health Improvement Navigator tofacilitate thecommunity health needs assessment and development of plans to address identifiedneeds (Roy, 2016). The goal is to assist hospitals, their associated health care organizations and COMMUNITY RESOURCES3community partners in identifying relevant interventions that they can adapt to have the greatestimpact on improving their community's health and well-being. The system focuses mostly on theseven health risk factors related to the most common causes of morbidity and mortality in theUnited States, but the design is adaptable to incorporate other important community healthissues.In addition, the interventions can be further filtered to properly address modifiable healthoutcomes and barriers (e.g., socioeconomic factors, physical environment, health behaviors, andclinical care).In the 6|18 Initiative, CDC and partners - like healthcare providers, public health workers,insurers, and employers who purchase insurance - targetsixcommon and costly healthconditions with 18 proven interventions. This initiative improves health and control healthcarecosts by giving partners evidence about high-burden health conditions and related interventions,highlights disease-prevention interventions to increase their coverage, use, and quality andaligning proven preventive practices with value-based ways of paying for healthcare (CDCorganization, 2020).These initiatives involve interventions that require collaboration, addressing communityhealth by focusing simultaneously on multiple determinants of health to allow equal opportunityof improving health and reducing disparities in health outcomes. They also provide informationso partners can make decisions that improve people's health and help control costs.Impact of Funding Sources, Policy, and Legislation on the Organization's ServiceProvisionThe CDC promotespublic health policies that improve in addition to protect thecommunities' health. The CDC health policy series are designed to provide practical guidance tostate and local public health practitioners, highlighting specific opportunities for public health to COMMUNITY RESOURCES4engage with health care to improve population health (CDC organization, 2020). The CDCworks with Public Health Laws, thegoal is to enhance the public health system's legalpreparedness to address emerging threats, chronic diseases, and other national public healthpriorities. These laws have important consequences for the health of defined populations. Theyderive from federal and state constitutions; statutes, and other legislative enactments; agencyrules and regulations; judicial rulings and case law; and policies of public bodies (CDCOrganization, 2020). The CDC and their partnersare working toward legal preparednessthroughout the public health system, developing and deploying new legal tools that policymakersand front-line practitioners will apply to the entire spectrum of 21st-century public healthchallenges and opportunities. Public health laws protect and improve communities by preventingepidemics and the spread of disease; promoting healthy lifestyles for children and families;protecting against hazards in homes, work, communities and the environment; ensuring high-quality health-care services; safeguarding and improving the quality of the environment; andpreparing for and responding to emergencies.The CDC acts on public health emergencies and requires rapid coordinated responsesamong Congress, government agencies, and partners. These entities fund policies and aim inachieving the CDC mission, while the CDC commits to the Pledge of the American People,being a diligent steward of the funds entrusted to the Agency. Treatment of chronic diseaseaccounts for an estimated three quarters of U.S. health care spending (Schmidt, 2016). Chronicdiseases directly affect overall health care budgets, employee productivity, and economies. TheCDC use thePresident's Budget request and contracts to purchase goods and services such assupplies, vaccines, Information Technology (IT), and equipment to advance the agency's publichealth mission domestically and abroad.These contract actions totaled $5.7 billion supporting
