Problems in Healthcare in Zuyderland Region:

 Dejuvenation (=young people leaving the region will lead to a shortage in healthcare
workers) and double aging (=people will get older)
 Unhealthy lifestyle: high multi morbidity (chronic diseases e.g.) which will increase further
until 2040.
 High healthcare utilization and costs. (due to low self-reliance and need of medication)

Overview on important matters in healthcare organizations:

 Focus on effective and affordable care
 Setting incentive right
 Regional and supraregional network care
 Equality of management and medical specialists
 Patient Participation

You can also look which care could be delivered at home in order to make new innovations in
Healthcare. For example, nursing days could be easily done at home in most of the cases instead of
the hospital.

Every patient needs a specific treatment plan. Patients can be divided into 3 groups:

 One time care (hernia patient)
 In a short periode a lot of care (breast cancer)
 Regularly care (hip, stroke patient -> care consumer and low self-sustainability)

Care models in Zuyderland Hospital:

 Focus Clinic: elective care, aimed at operational excellence, often monodisciplinary (for
patients with regular care)
 Integrated Practice Unit: complex, multidisciplinary care, focuses on patient care pathways
 Network care without boundaries: integrated care in a chain for the elderly in a vulnerable
position and the chronically ill. To prevent that they come into the hospital.

Long-term collaboration with health insurer to make affordable healthcare.

Innovation:

 Digitalization: eHealth, Digital consultations and video consulting, virtual reality and lenses
(when a patient is scared to come into a machine they will be reassured)
 Network care: admissions and treatment at home (centre between medical centre and GP ->
1.5 line centers where GPs and medical specialists are close to each other, e.g. for an referral
of an patient)
