Drug Template Drug: Bethanechol (Urecholine, Duvoid) Atropine (Atro-Pen) Neostigmine (Prostigmin) Lidocaine Propofol (Diprovan) Therapeutic Class: Urinary Tract Stimulant Antidote for anticholinerase poisoning Prevents breakdown of ACh Anesthetic Anesthetic Pharmacologic Class: Cholinergic (Muscarinic Agonist)- aka parasympathomimetic agents Anticholinergic (Muscarinic Antagonist) Reversible Cholinesterase Inhibitor Amide Action: Selective agonist at muscarinic cholinergic receptors Occupies muscarinic receptors to block parasympathetic actions of Ach and induces symptoms of fight or flight Reversibly binds to acetylcholinesterase to prevent the breakdown of Ach; thus more Ach is available to activate cholinergic receptors (skeletal muscle stimulation & activation of peripheral muscarinic receptors Inhibits transport of Na+ ions across neuronal membrane -Unconsciousness develops within 60 seconds and lasts 3-5 minutes -Promotes the release of GABA (main inhibitory neurotransmitter-> generalized depression of CNS) Indications (Uses): Urinary Retention (causes bladder emptying), GERD (increased esophageal motility and pressure in lower esophageal sphincter) and post-op abd distention(increased tone and motility of smooth muscle) -Treat hypermotility diseases of GI tract (IBS) - Suppress secretions during procedures - Increase HR in pt with bradycardia - Dilate pupils during eye exam -Muscarinic Agonist/ Mushroom poisoning - Myasthenia Gravis (fluctuating muscle weakness and predisposition to rapid fatigue; think one eye drooping from weakness) ~By decreasing the breakdown of ACh neostigmine makes it more available which -All forms of local anesthesia -Antiarrhythmic for Vfib/V-tach. -Sedative hypnotic for the induction and maintenance of analgesia (Mechanical vent
