His past medical history is: allergic rhinitis, depression, seasonal allergies, and hypothyroidism. C.D.’s father died at age 49 from an acute myocardial infarction (AMI). His mother has diabetes mellitis (DM2) and hypertension (HTN); and his brother died at age 20 from complications from cystic fibrosis (CF). C.D. has two young sisters that are alive and well. C.D. has been married for 25 years and lives with his wife and two children, he is an air traffic controller at the local airport. C.D. smokes a pack of cigarettes a day for the past 15 years, and drinks several beers every evening after work to relax. He does not pay attention to sodium, fat, or carbohydrates in the foods he eats; and admits to “salting almost everything he eats, sometimes before tasting it”. C.D. denies having dieted or exercised.
